How do I check that a plumber is licensed in Illinois?

Who is responsible if the work goes wrong?

The contractor you hire. Your agreement is with that professional, not with PlumberIllinois. That is why every page here links to the licence register and recommends getting the scope in writing.

What warranty applies to the work?

Whatever the contractor offers in writing — PlumberIllinois is not a party to the agreement. Ask for labour and parts warranty terms in the quote, and keep the licence number with the paperwork.
